Anterior Humeral Line

Introduction
- Key line demonstrating normal elbow alignment
- Should be used whenever reading a pediatric elbow radiographic to exclude occult supracondylar fracture
- Done on standard lateral view
Interpretation
Measurement
- Line drawn down the anterior surface of the humerus should intersect the middle third of the capitellum
Interpretation
- During a FOOSH, radial head impacts capitellum
- Often resulting in a supracondylar fracture with posterior displacement
- Minor displacement can push the humeral line posterior to the capitellum
Exceptions
- In children under 4, the anterior humeral line can pass through the anterior 1/3 of the capitellum
