Bicycle Test of Van Gelderen

Purpose

  • Used to help differentiate between vascular and neurogenic causes of leg pain (intermittent claudication)

Description

  • The patient is seated on the bicycle, peddling in an upright position
  • They then can alternate to the forward leaning position
  • Positive for vascular etiology
    • Claudication in both positions
  • Positive for spinal etiology
    • Reproduced while upright, relieved while leaning forward
  • Both submaximal and maximal effort may be used

Pathology

  • Neurogenic Leg Pain
  • Vascular Leg Pain
