Purpose

• To evaluate for Thoracic Outlet Syndrome
• This test was first described in service members who wore a heavy ruck sack for a long period of time

Description

• Patient is standing, examiner behind patient
• Examiner palpates radial pulse and applies downward traction at the shoulder
• Simultaneously, the patient "stands at attention", engaging their chest
• Positive Test:
  • Change in pulse intensity
  • Increase in neurological symptoms on affected side

Modification

• The patient is seated
• The patient is asked to retract shoulders posteriorly, depress shoulders inferiorly
• Patient then 'protrudes' chest anteriorly for up to 1 minute
• Examiner palpates radial pulse of affected limb
• Positive test
  • Reduction or obliteration of radial pulse due
  • Worsening symptoms
