Dorsopalmar Stress Test

From WikiSM

Other Names

Left wrist is shown in all photos. Arrows indicate direction of stress application. A Normal wrist showing application of dorsaldirected stress to the distal ulna with the forearm fully pronated to assess integrity of deep palmar radioulnar ligament. B Unstable wrist demonstrating marked translation of distal ulna with dorsal-directed stress indicating incompetent deep palmar radioulnar ligament. C Normal wrist showing application of palmar-directed stress to the distal ulna with the forearm fully supinated to assess integrity of deep dorsal radioulnar ligament. D Unstable wrist demonstrating marked translation of distal ulna with palmar-directed stress indicating incompetent deep dorsal radioulnar ligament.[1]

Purpose

Description

  • Requires examination in neutral, pronation and supination
    • Compare ulnar translation on the injured side to uninjured site
  • Pronated forearm
    • Examiner applies a dorsally directed stress to the distal ulna, stabilizing the radius
    • Laxity indicates injury to the deep palmer radioulnar ligament
  • Supinated forearm
    • Examiner applies a palmerly directed stress test to the distal ulna, stabilizing the radius
    • Laxity indicates injury to the deep dorsal radioulnar ligament
  • Normal test
    • Firm end points without side-to-side laxity compared to unaffected limb
