General
- One of the major synovial plicae of the knee
- Most commonly symptomatic, although not as anatomically present as others[3]
- Can be continuous or separate from the suprapatellar plica
- Highly variable anatomy in terms of transverse width, lateral extent
Sakakibara Classification
- Based on size and morphology
- Types A:consists of a cordlike elevation in the synovial wall, unlikely to be symptomatic
- Type B: has a shelflike appearance but does not cover the anterior surface of the medial femoral condyle, unlikely to be symptomatic
- Type C: medial patellar plica covers part of the articular surface of the femoral trochlea, more frequently associated with symptoms
- Type D: fenestrated and more likely to be symptomatic, relatively rare
MRI Features
- Best seen on T2 imaging with arthrography
- Appears as a small band extending from the medial aspect of the joint
