General Description
- 6 Tunnels which transmit the extensor tendons of the forearm into the hand
- Located on the dorsal wrist
- Each tunnel is lined internally by a synovial sheath
- The tunnels are separated from each other by a fibrous sheath
Compartment 1
- Located on the lateral or radial aspect of the wrist
- Tendons contained
- Associated Pathology
- Note: In about 1/3 of patients, the EPB tendon is compartmentalized off from the APL tendon[6]
Compartment 2
- Tendons contained
- Separated from compartment 3 by Listers Tubercle
- Bony prominence on the distal radius

Anatomic Landmarks
1st and 2nd Dorsal Compartment Intersection
- Compartment 1 crosses obliquely and superficially over compartment 2
- Angle is approximately 60 in the distal forearm
- Approximately 4-6 cm proximal to Lister's tubercle
2nd and 3rd Dorsal Compartment Intersection
- The 3rd dorsal compartment crosses superficially to the 2nd dorsal compartment
- This happens at approximately Lister's tubercle

Description
- Strong, fibrous band, extending obliquely downward and medially across the back of the wrist
- Thick portion of the antebrachial/forearm fascia
- Responsible for holding extensor tendons in place
- There are six separate synovial sheaths (described above)
Histology
- Deepest layer: gliding layer composed of hyaluronic acid secreting cells
- Middle layer: thick, consists of elastin, collage and fibroblasts
- Superficial layer: loose connective tissue, vascular channels
