Description

Proper and accessory UCLs reciprocally tlghten as the MCP joint extends and flexes. On extension (a) the proper UCL is lax and the accessory ligament is taut; in flexion (b) the proper UCL is taut and the accessory ligament is lax[2]

Anatomy

Proper Collateral Ligament (PCL)

Accessory Collateral Ligament (ACL)

  • Origin just palmer to the PCL
  • Runs parallel to the PCL and also inserts on the proximal phalanx and volar plate

Function

  • Prevents abduction or valgus/ radial deviation of the thumb at the 1st MTPJ
  • Ensure the ulnar and volar stability of the base of the thumb
  • UCL Proper: tight in flexion, relaxion in extension of MCPJ
  • Accessory UCL: tight in in extension, relaxes in flexion
