Purpose
- Evaluate the integrity of the Ulnar Collateral Ligament
Description
- Patient is seated or standing
- Elbow and shoulder are flexed to 90°, forearm is supinated
- The shoulder should be externally rotated (throwing position)
- Physician pulls on the patients thumb
- Optionally, the patient can be moved across a range of motion
- Positive test:
- Apprehension, instability, pain
Modified milking maneuver
- The patient is seated with the arm positioned at 70° of flexion, adduction, and maximal external rotation.
- This position is supported by the patient’s other arm.
- The examiner palpates the medial joint line with their thumb
- Fully supinate the patient’s forearm and pull on the patient’s thumb exerting a valgus force to the elbow.
- Positive test
- Apprehension
- Instability
- Medial joint pain.
